BackgroundHuntington’s disease has a poor prognosis. For HD patients in the Netherlands, one way of dealing with their poor prognosis is by drawing up an advance euthanasia directive. Little is known about the perspectives of HD patients on their AED.AimTo gain insight into patients’ views on and attitudes towards their AED, and changes over time.MethodsA longitudinal qualitative interview study using 1 to 6 semi-structured interviews over a period of maximum three years. Nine HD patients who either had an AED or (...) were thinking about drawing it up participated in this study.ResultsWe identified two themes that characterize patients’ perspectives on their AEDs: general character of the AED; uncertainty around their AED. Ad The conditions that the participants described in their AED were generally not very specific for the person. Mostly they were general notions of unbearable suffering. Familiarity with HD in the family could play a role in drawing up an AED. Ad Participants generally were aware of the tentative character of their AED and could have doubts concerning their own willingness or the willingness of others in the future. Sometimes these doubts were so great, that it prevented them from drawing up an AED. However, patients did not alter their AED during the follow-up period or changed in their view or attitude on their AED.ConclusionHD patients that draw up an AED usually describe general conditions for euthanasia and recognize that these conditions may change as the disease progresses. An AED or the wish to draw one up may be a good conversation starter for conversations about goals and preferences for future care. (shrink)

Though ambiguities abound, Scheurer was ahead of most of his scholastic contemporaries in that he tried to come to grips with Kant and to see in Kant someone more than the malin genie of modern philosophy. El artículo presenta una reflexión sobre la manera en que en los últimos veinte años la promoción de un determinado tipo de solidaridad en Chile ha contribuido a la conformación de una gubernamentalidad liberal avanzada, necesaria para la instalación de un programa neoliberal. La reflexión se enmarca en los aportes teóricos de Michel Foucault y tiene por objeto empírico piezas de publicidad de promoción de la solidaridad emitidas en Chile entre los años 2009 y 2010, que han sido analizadas en (...) el contexto del proyecto Fondecyt 1090534. Se presentan tres tipos de resultados: (a) se describen los sectores sociales que se construyen como agentes de la solidaridad, (b) se reflexiona sobre las prácticas solidarias más promovidas y (c) se indaga en la forma en que se interpela a los sujetos a ser solidarios. Los estudios sobre el Hadiz desde un punto de vista literario son escasos y se ocupan en buscar las influencias judías y cristianas. El objetivo final de este trabajo seria, reconocer las diferentes Técnicas Narrativas utilizadas en los llamados hadices naturales.

The ternary xV2O5–(40 − x)Li2O–60B2O3 glass system, where x = 1, 2, 3, 4 and 6 mol%, was prepared by normal quenching. Ultrasonic velocities and attenuation were measured at room temperature using a pulse-echo technique. Various parameters, such as elastic moduli, micro-hardness, Poisson's ratio and Debye temperature, were determined from the measured densities and velocities. The composition dependence of these parameters, in addition to the glass-transition temperature, suggested that vanadium ions were incorporated into these glasses as a network modifier, resulting (...) in the reconversion of BO4 tetrahedra to BO3 triangles by the breaking of B–O–B linkages and the formation of nonbridging oxygens (NBOs). The outcome was a reduction in network connectivity and rigidity with increasing V2O5 content. The results are explained quantitatively in terms of fractal bond connectivity, average atomic volume, network dimensionality, packing density, number of network bonds per unit volume, cross-link density and atomic ring size. The Makishima and Mackenzie model appears to be valid for the studied glasses when the fate of BO4 tetrahedra and creation of NBOs are taken into account. (shrink)

En répondant aux questions: qu'est-ce qu'on étudie?, comment on étudie? pour quel but on étudie? nous découvrons l'accroissement de la proportion du moment subjectif ce qui permets à dresser un système globale des sciences.

We report the identification of a recurrent, 520-kb 16p12.1 microdeletion associated with childhood developmental delay. The microdeletion was detected in 20 of 11,873 cases compared with 2 of 8,540 controls and replicated in a second series of 22 of 9,254 cases compared with 6 of 6,299 controls. Most deletions were inherited, with carrier parents likely to manifest neuropsychiatric phenotypes compared to non-carrier parents. Probands were more likely to carry an additional large copy-number variant when compared to matched controls. The clinical (...) features of individuals with two mutations were distinct from and/or more severe than those of individuals carrying only the co-occurring mutation. Our data support a two-hit model in which the 16p12.1 microdeletion both predisposes to neuropsychiatric phenotypes as a single event and exacerbates neurodevelopmental phenotypes in association with other large deletions or duplications. Analysis of other microdeletions with variable expressivity indicates that this two-hit model might be more generally applicable to neuropsychiatric disease. © 2010 Nature America, Inc. Structured interviews were held with 149 registered nurses in seven countries in America, Asia, Australia and Europe concerning the feeding of severely demented patients who do not accept food. The most common reasons for nurses being willing to change their decision to feed or not to feed were an order from the medical head, a request from the patient's husband and/or the staff meeting. There was a connection between the willingness to feed and the ranking of ethical principles. Nurses who (...) were most prone to feed the patient most often gave a high rank to the ethical principle of sanctity of life, while those who primarily chose not to feed the patient gave a high rank to the ethical principle of autonomy. All nurses stressed the ethical principle of beneficence. Des interviews structurés ont eu lieu avec 149 infirmiers/ères dans sept pays en Amérique, Asie, Australie et Europe concernant l'alimentation des malades gravement dément qui refusent de manger. La raison la plus générale pour des infirmiers/ères d'être prêt à changer leures décisions de donner à manger ou non sont: un ordre du médecin, la demande du mari de la malade ou de la réunion du personnel. Il y avait un lien entre la volonté de nourir et le rang des principes éthiques. Les infirmiers/ères les plus enclins de nourir la malade le plus souvent donnaient un rang supérieur au principe éthique de la sainteté de vie, pendant que ceux et celles qui choisissaient de ne pas nourrir la malade donnaient un rang supérieur au principe éthique de l'autonomie. Tous insistaient sur l'importance du principe éthique de la bienfaisance. Konstruktive Interviews wurden mit 149 ausgebildeten Krankenschwestern und Pfleger in sieben Ländern in Amerika, Asien, Australien und Europa gehalten über die Ernährung von schwer von Dementia praecox leidenden Patienten, die das Essen verweigern. Die gewöhnlichsten Gründe des Pflegepersonals für die Bereitwilligkeit, ihre Entscheidung, zu ernähren oder nicht, zu ändern, waren Anordnungen vom medizinischen Chef, Anfragen vom Ehemann der Patientin und/oder einer Personalsitzung. Es bestand ein Zusammenhang zwischen der Bereitwilligkeit zur Ernährung und dem Rang der ethischen Prinzipien. Die Pflegenden, die sich am meisten neigten, die Patientin zu ernähren, gaben dem ethischen Prinzip der Heiligkeit des Lebens einen hohen Rang, während die, die meistens vorzogen, die Patientin nicht zu ernähren, gaben dem ethischen Prinzip der Autonomie einen hohen Rang. Alle Pflegenden legten grossen Wert auf das ethische Prinzip der Wohltätigkeit. (shrink)

We investigate and analyze the dynamics of hepatitis B with various infection phases and multiple routes of transmission. We formulate the model and then fractionalize it using the concept of fractional calculus. For the purpose of fractionalizing, we use the Caputo–Fabrizio operator. Once we develop the model under consideration, existence and uniqueness analysis will be discussed. We use fixed point theory for the existence and uniqueness analysis. We also prove that the model under consideration possesses a bounded and positive solution. (...) We then find the basic reproductive number to perform the steady-state analysis and to show that the fractional-order epidemiological model is locally and globally asymptotically stable under certain conditions. For the local and global analysis, we use linearization, mean value theorem, and fractional Barbalat’s lemma, respectively. Finally, we perform some numerical findings to support the analytical work with the help of graphical representations. (shrink)

Los soberanos de la dinastía Naṣrí de Granada se intitulan sultanes o Amīr al-Muslimīn en su abundante correspondencia exterior con otros príncipes musulmanes o cristianos. Sin embargo, a partir de la segunda dinastía, comienzan a utilizar también el título califal aunque parece que de forma privada, dentro del entorno familiar o el cortesano, tal como prueban determinados documentos emanados del Dīwān al-Inšā’ naṣrí. El califato ostentado por los Naṣríes no es el mismo que el de los Omeyas o los Almohades (...) que llevaba emparejado el título de Amīr al-Mu’minīn y Jalīfat Rasūl Allāh, sino que lo acompañan del título de Jalīfat al-Raḥmān o Jalīfat Allāh, como otros soberanos musulmanes de la baja Edad Media. Su legitimidad está basada en una genealogía que les hace entroncar con los Jazārŷ, es decir con los Anṣār de Sa‘d b. ‘Ubāda, que pretendió proclamarse Califa a la muerte de Muḥammad. (shrink)

Encadrant le Sophiste, le Théétète et le Politique, pris tous les deux ensemble, soulèvent des questions et suggèrent des réponses quant à la double relation entre le philosophe et le politique, d’une part, et entre Socrate et l’Étranger d’Élée, d’autre part. Je montre ici que certains aspects de la Digression du Théétète sur la nature des philosophes servent à critiquer les questions que pose LaRépublique ainsi que les réponses qu’elle donne sur la philosophie et la politique . Cette critique prépare (...) ainsi le terrain pour les différentes analyses qui sont données dans le Politique, dialogue qui sépare l’enquête sur la royauté ou la politique, des réflexions sur le bonheur, précisément de cette façon que réclame le philosophe dépeint par le Socrate de la Digression. J’avance que Socrate ne pouvait lui-même proposer l’idéal du vrai politique, bien que certains éléments du Théétète en signalent le besoin. Cet idéal peut bien être vécu par un philosophe ; toutefois devenir un politique transforme en profondeur le philosophe , au point que le Politique estime plus correct de le définir comme un politique plutôt que comme un philosophe. Je suggère finalement, en m’appuyant sur ce qui précède, que Socrate et l’Étranger d’Élée sont complémentaires, en ce que tous deux sont engagés dans la philosophie, sans qu’aucun des deux ne puisse la définir ou l’incarner complètement.Considering the Theaetetus and Statesman together – as the two dialogues explicitly said to flank the Sophist – both raises and suggests answers to questions about the relationship between philosopher and statesman, and the relationship between Socrates and the Eleatic Stranger. It is argued that aspects of the Theaetetus « Digression » on the nature of philosophers serve to criticise the questions raised and answers given about philosophy and statecraft in The Republic and so to introduce the different analysis of these matters which will be given in the Statesman, which separates its inquiry into kingship or statecraft from its reflections on happiness in just the way called for by Socrates’ philosopher in the « Digression ». It is then argued that the ideal of a true statesman is one which Socrates could not have advanced, although aspects of the Theaetetus signal the need for such an ideal. And that ideal is one which may be lived out by a philosopher, but in becoming a statesman, crucial aspects of that philosopher are redefined such that the Statesman suggests that it is more correct to define him as a statesman than as a philosopher. Finally, it is suggested drawing on aspects of the preceding that the relationship between Socrates and the Eleatic Stranger is complementary in that both are committed to philosophy, although neither can define or exhibit it fully. Résumé : Le PPBS a été conçu comme un instrument destiné à ceux qui décident de l'allocation des ressources publiques : il doit leur permettre de prendre de meilleures décisions. S'il est très difficile de repérer et de juger les choix que le développement du système a inspirés aux Etats-Unis, il n'en est pas mains certain que ses résultats tangibles sant maigres car la masse des anciens programmes, principale source potentielle de moyens nouveaux, n'a guère été remise en question.La valeur (...) intrinsèque du PPBS ne peut être mise en cause. Ce qui doit l'être, c'est la lenteur de son développement, qui demeure peu avancé. Cette lenteur s'explique, car le système ne peut produire des fruits dans une terre stérile qu'on n'a pas préparée.La brutalité et les maladresses de l'opération déclenchée en 1965 dans le gouvernement fédéral américain ne pouvaient que renforcer la résistance au changement et retarder, sinon compromettre, le succès final. Les perspectives demeurent heureusement favorables, puisque la nouvelle administration ne semble pas décidée à faire machine arrière et que les Etats et Villes ont démarré sous de bien meilleurs auspices.La principale conclusion qu'imposent les expériences américaines est que toute juridiction intéressée par le PPBS doit impérativement développer, de la façon la plus générale, la volonté d'instaurer le système et s'assurer les moyens de le faire. C'est dire qu'il est peu sage d'engager la bataille sur tous les fronts à la fois : il faut tenter des percées. Mais c'est dire aussi combi en l'information et la formation sant essentielles. A cet égard, l'installation généralisée de certains éléments du système, à savoir la structure de programmes et le budget-programme, loin de présenter des inconvénients, peut constituer un excellent exercice en dehors de son utilité directe pour le preneur de décision. La bataille en cause est surtout celle de la conviction: l'outil est prêt, il est bon, encore faut-il que l' ouvrier accepte de s'en servir.Samenvatting:Planning-programming-budgeting werd ontworpen als een instrument voor diegenen die beslissen over de toewijzing van de overheidsmiddelen.Dit instrument moet de beslissers in staat stellen betere beslissingen te treffen. Het is niet makkelijk deze beslissingen op te sporen en te beoordelen welke in de Verenigde Staten getroffen werden onder invloed van de ontwikkeling van deze systeembenadering. Toch is het met vrij grote zekerheid te stellen dat de tastbare resultaten gering zijn omdat de veelheid aan oude programma's die tenslotte toch de voornaamste potentiële bronnen zijn van nieuwe middelen, nooit werden aangevochten.De intrinsieke waarde van PPBS kan daarom evenwel niet worden genegeerd. Wat wel dient aangeklaagd is de traagheid van zijn aanslepende ontwikkeling. Deze traagheid is te verklaren door het feit dat het systeem slechts vruchten kan afwerpen zo het ingeplant wordt in degelijk voorbereid terrein.De brutaliteit en de onhandigheden bij de operatie die in 1965 werd ingezet in het amerikaans federaal overheidsbestel konden niets anders dan de weerstand tegen de verandering versterken en afbreuk doen aan het uiteindelijk slagen van het systeem. Gelukkiglijk blijven de vooruitzichten gunstig want de nieuwe beleidsvoerders sinds de presidentsverkiezingen, blijken ermee te willen doorgaan en de staten en lokale overheden zijn ermee van wal gestoken onder veel betere omstandigheden. Het doorslaggevend besluit uit de amerikaanse ervaringen terzake is wel dat elke instantie welke belangstelling heeft voor PPBS, nodig moet op zo breed mogelijk vlak de wil doen groeien om het systeem te ontwikkelen en er ook de nodige middelen toe ter beschikking stellen. Dit betekent tevens dat het weinig aan te bevelen is de strijd op alle fronten terzelvertijd aan te binden maar wel geleidelijk doorbraken te verwezenlijken. Dit brengt dan evenwel ook met zich dat informatie en vorming essentieel zijn bij deze strategie. In dit verband moet dan ook gezegd worden dat reeds het invoeren van sommige deelementen van het systeem, zoals de programmastructuur en het programmabudget, voor de beslisser nuttig kan zijn voor zijn persoonlijk inzicht en vorming.De te voeren actie dient nog slechts te worden toegespitst op het overtuigen van de verantwoordelijken. Het instrument is immers gebruiksklaar en degelijk, er is alleen nog het wachten op de wil van de gebruiker dit instrument ter hand te nemen. (shrink)

La pandemia del Covid-19 provocó un cambio forzoso en la enseñanza universitaria, tanto a nivel metodológico como práctico. La Universidad de Granada ha creado un triple modelo basado en tres escenarios diferentes: virtual (e-learning), semipresencial (b-learning) y presencial. Los objetivos que pretendemos alcanzar consisten en presentar, analizar y difundir este modelo pedagógico creado para hacer frente a los desafíos futuros que puedan surgir. Para el proceso de análisis, se evaluarán la experiencia en el aula y los resultados obtenidos durante los (...) tres cursos académicos en los que se ha implantado este nuevo modelo en uno o varios de sus escenarios. From pre-Christian times until the present day, philosophers have discussed whether, given evil, belief in God can logically be maintained. Theists and non-theists remain unconvinced by one another's arguments. This study re-examines the question of God and evil from a neutral standpoint and claims that neither side has come to adequate grips either with the question itself or with the other side's case, chiefly because of failure to distinguish the kinds of problem raised by evil.

The interpretation implied by R. A. B. Mynors's Oxford text printed above is that the word simul in line 317 is a conjunction and that the expression refers to the final section of the race. The sense presumably is: ‘As soon as they trod the last stretch’ Nisus came out in front, whereas (...) previously the runners had all been in a bunch, pouring forth like a storm cloud from the start. This interpretation, which requires a full stop in the middle of line 317, was proposed by F. H. Sandbach in response to problems generally acknowledged in the line as traditionally punctuated, that is, with a lighter pause after similes and a full stop at the end of the line—as, for example, in F. A. Hirzel's Oxford text: effusi nimbo similes: simul ultima signant. With this punctuation the last three words of the line have been taken to mean either that the runners fix their eyes on the finish, which gives a meaning to the verb which is difficult in the absence of any reference to the eyes, or that other people apart from the runners mark out the finishing line, which gives unexceptional sense to the verb but an implausible timing to the operation: it is odd indeed to leave the marking of the finish until after the race has started. Sometimes metaphysicians appeal to simplicity as a reason to prefer one metaphysical theory to another, especially when a philosophical dispute has otherwise reached a state of equilibrium. In this paper, I show that given a Quinean conception of metaphysics, several initially plausible justifications for simplicity as a metaphysical criterion do not succeed. If philosophers wish to preserve simplicity as a metaphysical criterion, therefore, they must radically reconceive the project of metaphysics.
